   Which sounds least expensive to you? (A) The patient rides the mass transit bus to the doctor and says, "Doctor I have a sharp pain in my side, what can it be?" or (B) The patient in shock is brought to the ER in a "bus" (aka emergency vehicle), unconscious with a burst appendix and peritonitis? or (C) Just let him die screaming at home and drag his dead body to the street?
